Lifespace Communities is seeking an experienced MDS Coordinator to lead the Resident Assessment Instrument process and help ensure exceptional, person-centered care for our residents.

What You'll Do

As our MDS Coordinator, you'll play an important role in clinical quality, reimbursement accuracy, compliance, and resident care. You will:

  • Coordinate the community's MDS/RAI process in accordance with federal and state requirements.

  • Complete MDS assessments accurately and within required timeframes.

  • Coordinate admission, quarterly, annual, significant-change, and Medicare assessments.

  • Develop and oversee individualized care plans with measurable goals and resident-centered interventions.

  • Establish ARD schedules and partner with the interdisciplinary team to ensure timely completion of MDS sections and Care Area Assessments.

  • Lead care plan and weekly Medicare meetings and complete Medicare certifications and recertifications.

  • Support PDPM, PPS, Medicare, reimbursement, and documentation requirements.

  • Participate in resource utilization and triple-check processes prior to billing.

  • Monitor resident progress and identify changes that may affect care plans or PDPM classifications.

  • Ensure medical records, physician orders, diagnoses, assessments, and supporting clinical documentation are complete and accurate.

  • Transmit assessments and monitor validation through required CMS systems.

  • Generate and review CASPER reports and other regulatory and quality auditing tools.

  • Partner closely with nursing leadership, physicians, residents, families, and the interdisciplinary team.

  • Mentor and educate nursing team members on MDS, documentation, and care planning best practices.

  • Participate in QAPI and other clinical quality initiatives.

What You'll Bring

  • Nursing diploma or associate degree in nursing from an accredited program required; bachelor's degree preferred.

  • Current RN or LPN/LVN license as required by the state.

  • At least one year of MDS/RNAC experience.

  • Working knowledge of MDS, RAI, Medicare, care planning, and regulatory requirements.

  • RAC certification preferred.
