Short Description The Clinical Coordinator will be responsible for patient enrollment services associated with the digital remote monitoring programs (COVID-19, Heart Failure, COPD, Diabetes). Roles and responsibilities could include digital platform enrollment and alert monitoring, telephonic patient outreach and education, coordination and escalation of care, and communication and collaboration with the providers and multidisciplinary team. 1. Facilitates patient identification, screening, and enrollment in RPM program 15. Documents all events and communications in patients CUHC electronic health record 13. Documents vital signs weekly and when alerts out of range in the patient's CUHC electronic health record Experience Required 3-5 Years Preferred Education Requirements NJ LPN License and BLS American Heart Association Hourly Rate Min $33
Hourly Rate Max $53
