AccordCare is a licensed home care agency seeking experienced Field Registered Nurses (RNs) to complete Start of Care (SOC) admission visits in Broward County and South Palm Beach County.

  Complete Start of Care (SOC) admission visits

  
  
• 
  

  Provide prompt and accurate communication regarding client medical status to the Nurse Supervisor to support effective case management

  
  
• 
  

  Travel throughout Broward County and South Palm Beach County as needed

  
  
• 
  

  Demonstrate empathy, professionalism, and confidence when working with clients and families

  
  
• 
  

  Maintain thorough, accurate, and timely documentation

  
  
• 
  

  Anticipate potential issues and appropriately escalate or resolve concerns

  
  
• 
  

  Prioritize tasks and demonstrate a strong sense of urgency

  
  

• 
  

  Active Florida Registered Nurse (RN) license

  
  
• 
  

  Current Level 2 clearance in the AHCA Clearinghouse

  
  
• 
  

  TB test within the last 12 months or Chest X-ray within the last 5 years

  
  
• 
  

  Prior home care experience required

  
  
• 
  

  Excellent communication, teamwork, collaboration, and customer service skills

  
  
• 
  

  Ability to use an electronic health record (EHR) for documentation and care coordination
