Mosaic is looking for a *Licensed Practical Nurse (LPN)* to join our team! In this role, you will provide health care to individuals receiving services in the homes by scheduling medical appointments, assessing condition, reviewing medications, completing any necessary medical forms and by updating medical records. Requirements What YOU'LL Need: Licensed Practical Nursing/Vocational Nursing certification in good standing. Minimum of one year of LPN experience or 3 years of CNA/CMA required. - NEW GRADUATES WELCOME! Must be able to lift, push and pull at least 50 pounds.
