Why This Role Matters The Program Services Assistant plays a vital role in supporting individuals with disabilities to build skills, increase independence, and actively participate in their communities. This position provides hands-on, day-to-day support to Day Support Program consumers by assisting with activities, transitions, skill development, and personal care. Through consistent implementation of Individual Service Plan (ISP) outcomes, the Program Services Assistant helps ensure services are person-centered, respectful, and compliant with all regulatory requirements What You Will Do
- Provide direct support to consumers in accordance with written Individual Service Plan
- (ISP) outcomes.
- Monitor, document, and report daily consumer progress toward established goals.
- Maintain and follow daily schedules of training activities and services.
- Collect daily data and provide feedback to support outcome development and service planning.
- Communicate consumer achievements, incidents, progress, or concerns to the Program Services Specialist or Program Manager as needed.
- Assist with the creation and facilitation of specialized training activities.
- Perform morning and afternoon transition duties, including transportation assistance, personal care, and activity setup.
- Provide one-on-one support to consumers requiring additional or specialized assistance.
- Transport and support consumers during community-based activities such as fitness, recreation, volunteer work, shopping, dining, and educational outings.
- Support individuals in building community relationships and using the community as a learning environment.
- Uphold participant human rights and advocate for consumer choice, dignity, and independence.
- Deliver Day Support services in full compliance with HCBS Settings Regulations, ensuring:
- Community-integrated service environments
- Respect for privacy, dignity, and freedom from coercion
- Promotion of autonomy and informed choice
- Person-centered planning directed by the individual or their chosen representative
- Follow all safety protocols, including use of required personal protective equipment (PPE).
- Clean and disinfect program sites and vehicles daily, with frequent attention to high-touch areas.
- Complete and maintain all required Didlake training.
