Requirements

The ideal candidate will possess the following qualifications:



  • High School Diploma or GED required
  • Strong computer skills, including:

    • Microsoft Word and Excel
    • SPSS
    • Email systems, employee portals, and time-tracking systems (e.g., Teletime)


  • Strong organizational skills
  • Ability to work effectively as part of a team
  • Strong problem-solving and interpersonal skills
  • Ability to manage stress appropriately
  • Ability to use de-escalation techniques when needed
  • Ability to make sound decisions under pressure
  • Ability to manage multiple priorities simultaneously
  • Ability to work independently
  • Ability to work in confined and/or crowded environments

We are seeking Case Managers at the Riverview Wellness Village on State Road in Philadelphia, PA.



This role is full-time, second shift - Monday - Friday 4:00 PM - 12:00 AM.



Earn $23.32/hr.!



Position Details


The Case Manager will help residents living in the recovery house create goals focused on their recovery, improving relationships, obtaining an income, and securing housing. The Case Manager will meet with residents bi-weekly and has a caseload ratio of 35:1.



Core Case Management Responsibilities



  1. Client Support & Service Planning


    • Serve as the primary contact for assigned residents.
    • Conduct regular check-ins and maintain a consumer-driven, strength-based approach to recovery.
    • Develop and update individualized case management plans based on client needs.
    • Link clients to community resources such as medical care, mental health services, housing, employment, and recovery supports (e.g., 12-step meetings).


  2. Coordination & Advocacy


    • Coordinate services with external providers and monitor client progress.
    • Advocate for clients to ensure access to necessary supports and benefits.
    • Assist clients in obtaining valid identification and other essential documents.


  3. Documentation & Compliance


    • Complete intake documentation and maintain accurate, timely progress notes.
    • Ensure compliance with agency, state, and federal regulations related to recovery services.
    • Monitor record-keeping, authorized units, and ensure charts are up-to-date.



Additional Duties in a Recovery House Setting



  1. Medication Monitoring


    • Oversee medication monitoring and maintain accurate medication logs.
    • Ensure medications are stored securely and administered according to policy.


  2. Group Facilitation


    • Lead or co-facilitate recovery-oriented groups (e.g., relapse prevention, life skills, coping strategies).
    • Encourage participation and create a supportive group environment.


  3. Intake & Orientation


    • Conduct intake interviews and complete all required documentation.
    • Orient new residents to house rules, program expectations, and available resources.


  4. Crisis Intervention


    • Respond to emergencies or behavioral crises promptly and appropriately.
    • Document incidents within required timelines.


  5. Administrative & House Support


    • Assist with maintaining a safe, clean, and structured living environment.
    • Participate in team meetings, supervision, and quality improvement activities.
    • Perform other duties as assigned to support program operations.
