Section II: Summary of Job Function

The Oncology Care Transition Coordinator is a registered professional nurse who under the supervision of the Oncology Care Transitions Manager contributes to the care of any hospitalized cancer patients by engaging patients in the transitional care management process and by collaborating with the inpatient and ambulatory clinicians,

This person meets with patients upon admission to the inpatient unit, participates in multidisciplinary daily rounds, ensures the coordination and confirmation of appointments, follow up requirements, and needs for discharge,

Section III: Job Qualifications/Education/Training/Certification/Licensure

Education:

Baccalaureate degree or higher

Graduate of an accredited school of nursing

Currently licensed to practice professional Nursing in the State of New Jersey

BLS Certification Required

Previous oncology experience and OCN preferred

Experience: Knowledge of medical terminology and experience within a healthcare setting required

This position requires every other weekend
