The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Job Overview:

The Pharmacist is responsible for the provision of pharmaceutical care to all patient age groups (including geriatric, adult, adolescent, pediatric and neonatal patients). They are responsible for the preparation, dispensing and monitoring of both the prescribing and effectiveness of all medications and IV admixtures. They exercise considerable independent decision making.

The Pharmacist II is a Generalist performing as a Pharmacist I in a specific practice area where they use their expanded knowledge to enhance process and pharmacy services in this area. Examples include dedicated or primary pharmacist in outpatient cancer infusion center, inpatient oncology, pediatrics, infectious disease, NICU (neonatal intensive care unit), critical care, operating room, emergency department, transitions of care. May include expanded clinical research study involvement in this area.

Qualifications:

  • Graduate of an ACPE- accredited college of Pharmacy, or a foreign equivalent educational degree; BS. In Pharmacy or Pharm D required.

  • Advanced degree preferred.

  • Current Registration in New Jersey as a Licensed Registered Pharmacist

  • Experience:

  • Familiarity with all phases of Hospital Pharmacy.

  • Working knowledge of public health matters.

  • Familiarity with Federal and State laws regulating professional and hospital practice of Pharmacy.

  • Understands role of Hospital Pharmacy Department within the Hospital, its interrelationships with other departments and functions.

  • Successful completion of all orientations materials.

  • Successful Completion (90%) of all assigned departmental competencies.

Essential Functions:

Job Specific Requirements

Adjusts medication orders to comply with P&T approved substitutions, and converts TYPED-IN orders to formulary equivalents, if applicable.

Assumes role of drug information provider to all members of the health care team specific to patient populations and age groups.

Contacts physician for any medication orders that require clarification.

Correctly documents all controlled substance dispensing. Reports and resolves discrepancies.

Displays self initiative in anticipating and resolving daily work issues.

Documents interventions.

Enhances professional growth and development through participation in educational programs, current literature, workshops, inservice meetings, etc.

Follows proper safety and infection control procedures and maintains a safe work environment. Participates in the safety and infection control management programs.

Maintains the confidentiality, security and integrity for the management of associate/patient information according to policy and procedure.

Monitors specific medication prescribing in accordance with established drug usage criteria, prior to dispensing, as defined by the Pharmacy & Therapeutics Committee.

Notifies nurse of any changes/modifications of medications prescribed.

Participates in activities, committees, projects and teams.

Participates in medication error reporting programs Evaluates and reports Adverse Drug Reactions.

Provides direct supervision of all non-pharmacist personnel.

Provides education for pharmacy students on rotations as required.

Provides one (1) clinical presentation per year (e.g. journal article review, drug review, etc).

Signs cart fill sheet, completes unverified medication check and signs the sheet that they have been done.

Supervises technical staff and volunteers to ensure accuracy and compliance with policies and procedures.

The staff member demonstrates the ability to identify what the patient's educational needs are.

The staff member participates in the interdisciplinary patient teaching process.

The staff member prepares parental admixtures, dispenses, and compounds medications.

Utilizes the medication screening process to insure medication orders are complete, appropriate, within legal parameters and prioritized.

Willingness and flexibility to work alternating and rotating work schedules on short notice.

Willingness to work with the realization that errors may have serious consequences to patients.
