JOB RESPONSIBILITIES:

  1. Facilitate the development, implementation, and ongoing adherence of team-based, asynchronous, digital care model (TAG) protocols and therapies to assure that all individuals with intermediate to high ASCVD risk receive comprehensive and timely treatment and interventions that are consistent with best practice recommendations.
  2. Develop sustainable protocols that meet grant requirements.
  3. Monitor program performance measures including but not limited to:

    1. Patient volumes from initial entry to long-term chronic care initiatives
    2. Coordination with other partners to ensure wrap-around support to enrollees
    3. Inform grant partners of challenges and opportunities to develop a successful TAG model
    4. Work with SSCHC analyst to provide performance metrics


  4. Support the evaluative grant process in partnership with SSCHC grant department.
  5. Work with the multi-disciplinary team to build a strong team atmosphere, to foster continuous improvement in current and future TAG processes.
  6. Build sustainability into TAG processes and ensure coordination of care.
  7. Work directly with patients as needed for remote monitoring purposes and outreach with instructions when appropriate.
  8. Develop and deliver educational programs to help patients and their families learn more about HTN control and remote monitoring.
  9. Act as a liaison for and participate in grant-related meetings and other organizational projects as needed.
  10. May act as an RN team leader, supervising patient care in an ambulatory care setting.
  11. Participate as an active member of various meetings and committees including operations/managers meetings and others as needed.
  12. Work directly with VP of Operations, DON and other grant project leaders to grow an existing remote monitoring program into a sustainably higher level of performance and volume.


QUALIFICATIONS:

  1. Active Nursing license in Wisconsin
  2. Current BLS.
  3. Minimum of 2 years clinical experience with program management experience.
  4. Exhibits excellent interpersonal communication skills with compassion, empathy and sensitivity to participant needs.
  5. Desire and willingness to collaborate as a member of a high-functioning transdisciplinary team.
  6. Can work remotely as approved by SSCHC HIPAA team with in-person requirements as needed.
