Catholic Charities Maine is seeking a compassionate and highly skilledClinical Care Coordinatorto join ourAdult Opioid Health Home (OHH)team. In this vital role, you will support adults living with opioid use disorder and co-occurring mental health challenges by coordinating integrated behavioral and physical health services that empower individuals to reach their highest level of wellness and recovery.

About the Role:TheAdult Opioid Health Home (OHH) Clinical Care Coordinatorshall be a trained, supervised professional who will work as an integral part of the OHH Team to coordinate and monitor the delivery of effective, integrated behavioral and physical health services to enrolled OHH members. She/he will provide clinical support to include individual and group sessions focused in the areas of treatment planning, relapse prevention, safe approaches to harm reduction, and motivation along with coordination and management of the overall integration of the member's emotional and physical health supports and services toward assisting them in achieving their optimal health/wellness goals.

Starting Salary:$22.50 - $25.25 per hour. Actual starting salary contingent with experience/education.

Qualifications

What You'll Bring:



  • Education: Bachelor's degree in Psychology, Human Services, or related field.
  • Credentials: Current MHRT/C certification and CADC required.
  • Experience: Minimum 2 years working with adults experiencing mental illness and addiction in a clinical or community-based setting.
  • Skills:

    1. Strong knowledge of addiction, mental illness, and community resources.
    2. Proficiency in electronic health records.
