Overview

We are seeking a Social Worker, also referred to as a Clinical Social Worker or Behavioral Health Social Worker. The Social Worker will support patients and families by addressing emotional, social, and practical needs related to illness or injury. Medical Social Workers assist with discharge planning, crisis intervention, and connecting patients to community resources and services. Working as part of an interdisciplinary team, they help ensure comprehensive, patient-centered care.

Responsibilities

Essential Duties and Responsibilities (includes, but not limited to):

    Psychosocial assessments: evaluate the emotional, social, and environmental needs of patients to identify challenges affecting recovery and care
  • Discharge planning: coordinate safe and appropriate discharge plans, including referrals to home health, rehabilitation, nursing facilities, or community services
  • Crisis intervention: provide support during emergencies, such as trauma, end-of-life decisions, abuse, or mental health crises
  • Patient and family support: offer counseling, education, and advocacy to help patients and their families cope with illness, stress, or difficult diagnoses
  • Resource connection: link patients to financial assistance, housing, transportation, mental health services, and other community resources

Another big portion of this job is going to be Case Management:

  • Develop individualized care plans in collaboration with patients, families, and healthcare teams to optimize health outcomes.
  • Monitor patient progress and modify care plans as needed in response to changing health needs.
  • Advocate for patient needs and preferences within the healthcare system.


Qualifications

EDUCATION, EXPERIENCE, TRAINING

1. Bachelor's Degree Require, Master's Degree Preferred
2. In states requiring license, required upon hire and maintain current in accordance with state regulations. In states without a licensure requirement, current registration for with appropriate state board required and/or under clinical supervision to meet clinical hours required for licensure.
3. Current BLS certificate upon hire and maintain current.
4. Minimum one (1) year Inpatient l or Outpatient Behavioral Health experience. Experience in working with a diverse population with a wide range of problems (i.e., death and dying, geriatric psych care and subsequent diagnoses, substance abuse, disability, chronic illness, etc.).
