LPN - PT Nights

We are seeking a dedicated and skilled Licensed Practical Nurse (LPN) for a part-time night shift position. The role involves working 24 hours per week, with a schedule of three 8-hour shifts one week and two 8-hour shifts the following week, including every other weekend on Friday and Saturday nights.

• Valid New York LPN Licensure
• Basic Life Support (BLS) certification from AHA or RQI
• Minimum of 1 year experience as an LPN
• Experience in a Long-Term Care (LTC) facility is preferred

• Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse.
• Collect data relevant to residents' physical and psychosocial condition and communicate findings to the RN and other team members.
• Contribute to the development of a culturally relevant and age-appropriate care plan.
• Recommend revisions to the care plan based on evaluations and collaborate with residents, families, and the healthcare team.
• Maintain current knowledge and competency in nursing practice through continuing education and in-service programs.
• Participate in mentoring and orientation of new staff.
• Ensure safety, cost-effectiveness, and quality in the delivery of care.

Healthcare professionals must be available for assignment until January 19th. Holiday time-off requests cannot be accommodated, except one holiday off is guaranteed. Occupational exposure to bloodborne pathogens is reasonably anticipated in this role.
