Our range of health services include home care, hospice and palliative care for adults and children, rehabilitation and nursing care at Menorah and Isabella Centers, and the research based MJHS Institute for Innovation and Palliative Care. We also offer Elderplan/HomeFirst: health plans for Medicare and dual-eligible individuals. Evaluates patients referred for home health services. Manages and facilitates the transition of patients onto the home care program in accordance with applicable law, regulations and agency policies and procedures.

  • Associates in Nursing; Baccalaureate Degree preferred.
  • Minimum of one year of acute, Medical-Surgical nursing experience, preferred.
    Home Care experience preferred
  • Specialized knowledge in Microsoft Office, (Word, Excel, Outlook, etc.)
