Complex Care Registered Nurse

COMMUNITY SUPPORT SERVICES

FULL TIME - EXEMPT

General Summary: Provide in home clinical mental health services and nursing services to patients with complex psychiatric and medical needs.

Schedule: Mon-Fri, First Shift

Location: Warwick, RI

Salary: Starts at $80,000 annually*

Essential Responsibilities:



  • Provide in home comprehensive nursing assessments
  • Utilize comprehensive nursing assessments, plans, interventions and evaluations in direct client care to patients experiencing psychiatric and medical symptoms
  • Monitor medication and administer injections with strict adherence to Thrive policy and procedure
  • Monitor signs and symptoms of psychiatric functioning; monitor medication regimen; in community settings
  • Provide complex care assessments, services and ongoing short term case management planning psychotherapy services to adult outpatients
  • Consult/liaison with psychiatrist and/or medical doctors and other treatment staff
  • Develop and implement recovery care plans and document services consistent with Thrive standards and/or insurance requirements
  • Collaborate with other providers, agencies and individuals in the client's network of care to create linkages to services necessary on a long term basis
  • Provide education, training and modeling in basic daily living skills and self-care skills
  • Teach clients effective techniques for managing their medication as prescribed
  • Assist clients in developing and strengthening their support network to accomplish goals and objectives



Additional Responsibilities:



  • Attend all agency/department required training and meetings within the prescribed period.
  • Attend all Safety Training programs and be able to describe his/her responsibilities related to the general safety, department safety, and specific job-related hazards and responsibilities.
  • Demonstrate respect and regard for the dignity of all clients, families, visitors and fellow employees to ensure a professional, responsible and courteous environment.

Qualifications

Education, Experience and Competencies:



  • Bachelor's Degree in the Nursing or an equivalent combination of education and experience required
  • Minimum (2) year experience in a mental health setting working with clients with disabilities preferred.
  • Experience or training in related state and federal assistance programs also preferred.
  • Basic computer literacy skills are necessary.



Certifications, Licenses Requirements:



  • Rhode Island registered nursing license required.
  • Valid driver's license and automobile insurance which meets Rhode Island minimum standards required, or the ability to obtain necessary transportation in order to perform the responsibilities/tasks of the job.
