Positions with a designated work location in New York City, Nassau, Rockland, Suffolk, or Westchester Counties will receive a $4,000 annual downstate adjustment (pro-rated for part-time positions).

Job Description:

Responsibilities

The Health Program Coordinator I will coordinate Diabetes Program deliverables. Responsibilities include managing contracts; providing technical assistance to organizations achieving, maintaining and sustaining National Diabetes Prevention Program (National DPP) and diabetes self-management education and support program (DSMES) recognition and accreditation; supporting subcontractors' implementation of National DPP and DSMES programs in healthcare and community-based settings; developing and enhancing state and local partnerships for delivery of National DPP and DSMES programs; contributing to the development of subcontractor workplans and budgets; providing guidance for National DPP and DSMES Medicare and Medicaid billing and provider enrollment; facilitating monthly internal/external project team calls; contributing to evaluation and program narrative reports; developing program specific presentations and website content; and related activities. Other appropriate duties as assigned.

Minimum Qualifications

Bachelor's degree in a related field and three years of experience in program coordination in a public health, health, health regulatory, or human services related program; OR an associate's degree in a related field and five years of such experience; OR seven years of such experience. At least one year of experience must have included supervision of staff and/or program management. A master's degree in a related field may substitute for one year of such experience.

Preferred Qualifications

Graduate degree in public health, public/health care administration, social work, nursing, or related field. Experience in the development, delivery, coordination or implementation of evidence-based diabetes prevention and/or self-management programs in community-based organizations, health systems, state or local health departments, and/or federally qualified health centers. Experience in the management of state or federally funded contracts. Familiarity with Medicaid and Medicare policy and billing processes for the National Diabetes Prevention Program and/or diabetes self-management education and support programs. Experience coordinating diabetes public health initiatives with community pharmacies and/or community health workers.

Conditions of Employment

Grant funded position. Compliance with funding requirements such as time and effort reporting, grant deliverables, and contract deliverables, is required.

Valid and unrestricted authorization to work in the U.S. is required. Visa sponsorship is not available for this position.

Travel up to 10% of the time will be required.

The selected candidate must reside within a reasonable commuting distance of the official work location specified in the job posting and must also be located in or willing to relocate to one of the following states: New York, New Jersey, Connecticut, Vermont, or Massachusetts, prior to hire. Telecommuting will be available.
