Position Highlights |
We are seeking a dynamic, motivated Peer Navigator to join our team and support our mission to provide comprehensive services to individuals facing challenges. The Peer Navigator will play a crucial role in connecting clients with the resources and support they need to succeed through the Opioid Assistance and Referral Line (OAR). This position will require occasional travel to Tucson. Duties & Responsibilities |
- Provide one-on-one navigation and support to clients who request information and assistance through the OAR line (telephonic) and occasional bedside consultations.
- Assist clients in accessing community resources, including healthcare, recovery programs, and social services.
- Build trusting relationships with clients through consistent and supportive communication.
- Advocate on behalf of clients by addressing barriers, coordinating with providers, and ensuring fair access to services and resources.
- Support the implementation of individualized client plans by supporting goal setting, monitoring progress, and coordinating access to services under established guidelines.
- Provide feedback to the program team on common challenges clients' encounter.
- Maintain accurate, up-to-date records of client interactions, service referrals, and progress in accordance with requirements.
- Contribute to program evaluation by tracking service utilization and outcomes.
- Participate in team meetings and outreach activities to support program visibility and improvement.

Minimum Qualifications |
- Bachelor's degree or equivalent advanced learning attained through experience required.
- Up to 1 year of relevant work experience may be required.

Preferred Qualifications |
- Experience in peer support or related roles.
