Responsibilities: - Provide ongoing social work services to participants and their authorized representatives, assisting with personal, environmental, and health-related challenges - Educate participants and families on community resources, benefits, and services such as Medicaid, Medicare, housing, and food assistance - Coordinate and communicate care plans during and after hospitalizations or inpatient stays, ensuring participant wishes are respected - Offer social work consultation, including education on treatment options, palliative and end-of-life care, and arranging supportive services - Complete biopsychosocial assessments, cognitive/emotional status evaluations, and advance directives documentation - Develop and implement participant-centered care plans with clear psychosocial goals and interventions - Document all assessments and interventions accurately and timely in EMR systems - Manage workload to provide maximum social services, addressing social, financial, and psychological issues - Collaborate with healthcare professionals and community agencies to coordinate care and services - Facilitate psycho-educational presentations for participants to promote awareness and understanding of social work roles - Attend and actively participate in interdisciplinary team (IDT) meetings, contributing social work insights and recommendations - Engage in continuing education, professional development, and peer supervision to enhance skills and knowledge - Represent the social work department and organization at community meetings and conferences Qualifications: - Licensed Social Worker (LCSW, LMSW, or equivalent) required - Proven experience in providing psychosocial assessments and supportive counseling - Strong knowledge of community resources, healthcare benefits, and social services (Medicaid, Medicare, SNAP, housing, etc.) - Excellent communication and interpersonal skills for working with participants, families, and healthcare teams - Ability to conduct mental status assessments and develop individualized care plans - Experience coordinating care across multiple settings, including hospitals, SNFs, ALFs, and community agencies - Proficiency in documentation and electronic medical records (EMR) systems - Ability to manage a caseload effectively and prioritize tasks - Knowledge of crisis intervention and end-of-life care practices - Commitment to ongoing professional development and participating in team meetings and training - Strong collaboration skills within interdisciplinary teams and community organizations *Additional Salary Detail The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future.
