Medical Records Clerk - 1st Shift


Behavioral Health Services


Hourly Pay Range: $14.87 - $22.28

Job Summary: The role of the Medical Records Clerk is to ensure the completeness, accuracy, and privacy of all client's medical records. This position involves record control, client registration, insurance verification, release of information, HIPPA regulations, customer service, processing of legal documents and Electronic Medical Records (EMR). The Medical Records Clerk is responsible for routing and maintaining all medical information. This role will complete the registration function supporting Community Access to Recovery Services (CARS), Community Reintegration Center (CRC), Comprehensive Community Services (CCS), Comprehensive Community Mental Health Services (CCMHS), Matt Talbot Recovery Services, and any future providers in need of the function. The Medical Records Clerk will complete all fiscal insurance and consent information, entering the data into the Behavioral Health Services Electronic Medical Records system (Avatar). All staff are required to work holidays and weekends on a rotation schedule. Staff may be subject to mandating.



Essential Responsibilities:



  • Perform assigned clerical tasks as needed. Maintain the EMR files for all patients, including opening and closing of episodes of care, diagnosis, and entering fiscal insurance data. Enter and verify client information. Perform insurance verification, legal processing, and release processing.
  • Enter accurate client information into the system to open new accounts or episodes in existing client chart.
  • Add insurance information into Avatar and scan to chart. Update all demographics at this time.
  • All staff on duty cover reception and incoming phone calls, inquires, registration, release of information, direct calls, and take messages. Receive release of information requests. Complete registrations received via registration line. Update logs of all registrations and requests.
  • Assist public requests, process faxes, sort and distribute mail, and process alerts and record requests. Maintain and update fax log daily.
  • Manage outside scanning coming in the shared email. Granite Hills Hospital, CARS, CRC, Registration, CCS, CCMHS, Matt Talbot Recovery Services, and other facilities information pertaining to the medical record or insurance verifications.
  • Responsible for all incoming medical and legal documents processing.
  • Assisting providers, finding paperwork, assisting in completing records.
  • Knowledge of HIPPA rules and regulations. Following all guidelines in place.
  • Perform all other duties as assigned.



Minimum Requirements:



  • Current resident of Wisconsin.
  • Possession of a High School Diploma or GED Equivalent.
  • At least one (1) year of related work experience.
  • Basic Knowledge of all related computer and software applications, such as word processing and spreadsheets.
  • Basic Knowledge of EMR Scanning.
  • Able to lift/carry 20 lbs and push/pull 30 lbs.
  • Willing and able to work rotating holidays and weekends.



Preferred Qualifications:



  • Knowledge of HIPPA rules and regulations.
