We are seeking a Lead Intake Case Manager to join CTI's Youth Services team, located at the Youth Opportunity Center. The Lead Intake Case Manager will provide advanced levels of support to young adults (14-24) and their families. This includes providing advanced level case management, advocacy, referral information and supporting participants in overcoming barriers towards long-term, independent housing. The Lead Intake Case Manager will supervise and lead members of the Intake and Assessment team.

Responsibilities Include:



  • Oversight of Intake and Assessment pathways for all Youth Services programming, including referral tracking, connection request forms, and internal referral processes
  • Train and supervise Intake Case Management staff
  • Develop and use network of available services to assist participants and staff
  • Support and complete documentation and assessment tracking according to grant requirements
  • Lead and facilitate team meetings to address program and participant goals
  • Collaborate with Intake staff to support participants in housing goals and service planning
  • Establish and maintain working relationships with internal/external partners
  • Supervise and manage Intake Case Managers
  • Data input, file management, case management software
  • All other tasks and administrative duties as assigned


Qualifications:



  • Bachelor's Degree preferred, preferably in a Human Services related field. Minimum 5+ years' direct case management experience may substitute for degree
  • 2 years of experience working with youth and young adults
  • Ability to work comfortably with pregnant and parenting youth in both group and individual settings, preferably with experience advocating for families
  • Minimum of one year of assessment, advocacy and case management work experience in a Human Service field required
  • Demonstrated knowledge of community resources available to low-income individuals and families
  • Excellent customer service, organizational, oral and written communication skills
  • Ability to manage multiple priorities and work in a fast-paced environment
  • Proficient computer skills required in basic computer software and data collection systems
  • Excellent crisis management skills
  • Ability to maintain accurate records and confidentiality of information
  • Excellent attention to detail
  • Bilingual (English/Spanish, English/Khmer, English/Portuguese) preferred
  • Valid Driver's License
