The Care Coordinator will work in support of the Case Management and Medicaid Long Term Support Services (LTSS) teams, as well CDPHP members, providers, and community partners. Working collaboratively with Case Managers, the Care Coordinator will provide expertise in conceptualizing and implementing effective outreach and engagement initiatives, with a population that can be hard to reach. The Care Coordinator will assist with member and provider outreach calls, coordination of required provider visits and in-home assessments. Outreach may include but is not limited to, member communication about gaps in care and preventive health opportunities, member and provider education, communication campaigns, and medical management referrals for all lines of business. The Coordinator will assist with mail processing, materials organization, technology/service requests and other administrative tasks as necessary. At times, the Care Coordinator may travel locally as needed.

Qualifications:

  • Associates degree or two (2) years of equivalent experience required.
  • Minimum three (3) years of experience working in a direct human services capacity required.
  • Experience with Government or State Programs population required.
  • Experience with and knowledge of social determinants of health required.
  • Proficiency in Microsoft Outlook, Word, and Excel required. Knowledge or experience in PowerPoint, Access or other database software preferred.
  • Strong understanding of community characteristics and resources in the local community preferred.
  • Bilingual preferred.
  • Excellent verbal and written communication skills.
  • Demonstrated ability to pro-actively identify problems, as well as recommend and/or implement effective solutions.
  • Demonstrated ability to work with and maintain confidential information.
  • Demonstrated ability to provide excellent customer service and develop relationships both internally and externally.
  • Excellent verbal and written communication skills.
  • Flexibility to adapt to a changing and fast-paced environment.
  • Ability to travel locally as needed. Access to reliable transportation and a valid driver's license is required.

CDPHP salary ranges are designed to be competitive with room for professional and financial growth. Individual compensation is based on several factors unique to each candidate, such as work experience, qualifications, and skills. In addition to cash compensation, CDPHP employees may be eligible for an incentive payment, a discretionary cash reward based on employee and company performance. Some roles may also be eligible for overtime pay.
