Park Nicollet is looking to hire a heart failure (HF) transitions of care (TOC) Pharmacist to join our Methodist Hospital team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.

Position Summary:

The pharmacist will focus on patients admitted at Methodist Hospital with a primary diagnosis of heart failure. They will ensure the accurate and timely dispensing and safe and appropriate use of medications. This includes receiving and interpreting physician's orders to be dispensed, compounded and/or admixed for inpatients or outpatients. For outpatients, this will include patient counseling on proper medication use and potential side effects. These patients have medication history completed on admission, guideline based recommendations to optimize HF regimen, discharge medication history, meds to bed, bedside patient education and a follow up phone call within 72 hours of discharge. The position has the potential of evolving into more high risk disease states in the future (e.g. acute myocardial infarction).

Work Schedule:

Day shifts

40 hrs weekly / 1.0 FTE

Required Qualifications:



  • Bachelor's or PharmD degree in pharmacy
  • Completion of a PGY-1 Hospital Pharmacy Residency
  • Current licensure in Minnesota


Preferred Qualifications:



  • Previous experience working in a TOC capacity
  • Interest and/or additional training in cardiology
  • Completion of a PGY-2 Hospital Pharmacy Residency preferred
  • Institutional experience (hospital) is preferred.
  • Pharmacist registered in Minnesota who is familiar with pharmacy regulations and standards, medication delivery systems, information systems, automation, institutional practice and third party pay programs.
  • This highly organized individual should possess the ability to work effectively with others to efficiently deliver care for patients.
  • Leadership, written/verbal skills, and customer service skills are essential.
  • Intermediate level computer/keyboarding skills required.
  • Demonstrates knowledge of age-related growth and development principles necessary to provide appropriate service and assure safety to patients.
  • This position requires familiarity and compliance with matters of law, regulations and internal policies affecting the employment relationship, including, but not limited to, FMLA, ADA, discrimination and harassment laws, patient confidentiality, PNHS compliance programs, HCFA, CMS, TJC, FDA, DEA, Board of Pharmacy, OSHA, EPA and the Minnesota Department of Health.
