Position: RN Case Manager - New Graduate Program for Home Health

Salary: $68,000-$83,000 annually, depending on experience

Service Area: Lifespark Corporate Office, St. Louis Park 55416

Schedule: Full Time 1.0 FTE, Monday through Friday 8am-5pm

The RN Case Manager is responsible for planning, organizing and directing the Lifespark services while ensuring compliance with regulatory requirements and Lifespark best practices including but not limited to completing, admissions, care plan development, ongoing support, client satisfaction, caregiver supervision and engagement, and onboarding. The professional nurse builds from the resources of the community & care team to plan and direct services to meet the needs of individuals and families within their homes and communities.

Position Functions & Responsibilities:



  • Completes an initial assessment of client and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
  • Regularly re-evaluates client nursing needs.
  • Initiates the plan of care and makes necessary revisions as client status and needs change.
  • Uses health assessment data to determine nursing diagnosis.
  • Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the client and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures. Sets up and/or administers medications and treatments as prescribed by the physician. Provides direct client care as defined in the State Nurse Practice Act.
  • Counsel the client and family in meeting nursing and related needs.
  • Provides health care instructions to the client as appropriate per assessment and plan of care.
  • Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the client.
  • Acts as Case Manager when assigned by Clinical Manager and assumes responsibility to coordinate client care for assigned caseload.
  • Lead by example in establishing rapport with clients, clinicians, partners, and the community.
  • Ensure positive/professional communication within the team
  • Customer service resolution; escalate concerns
  • Prepares clinical visit notes daily and updates the primary physician when necessary and at minimum of every 60 days.
  • Communicates with the physician regarding the client's needs and reports any changes in the client's condition; obtains/receives physician's orders as required.
  • Communicate with community health related people to coordinate the care plan.
  • Timely completion of all documentation same day and any OASIS by 10am next day
  • Understands and complies with all the rules and regulations that govern the responsibilities of a registered nurse and home health
  • Completes orientation and annual training requirements assigned
  • Participates in Lifespark's Event Management process including initiating reports, escalation of events (vulnerable adult reports) and investigations
  • Instructs, supervises, and evaluates home health aide care provided up to every two (2) weeks
  • Manages client supplies by assuring appropriate DME provider is utilized.
  • Schedules and keeps member appointments
  • Organizes travel efficiently and documents mileage accurately within EMR
  • Maintain routine open communication and education with preceptor and clinical manager for duration of RN grad position
  • Work collaboratively with a preceptor to learn to case manage and care for a population of shared clients
  • A performance review will occur after orientation.
  • Other duties as assigned



Qualifications:



  • Registered nurse, BSN preferred, Current Minnesota Nursing License
  • Knowledge of Medicare Home Health preferred
  • Experience in customer service/direct member contact field preferred
  • Knowledge of corporate business management, governmental regulations, and private payer practices
  • Graduate of an accredited school of nursing
