Position: Clinical Manager-Excellence for Hospice

Salary: $100,000 - $110,000 annually, depending on experience

Schedule Monday - Friday 8:00am - 5:00pm

Service Area: Lifespark Corporate Office, St. Louis Park 55416

Schedule: Full Time 1.0 FTE, Monday through Friday 8am-5pm - (rotation: weekends, holidays and on-call with Admins)

The Clinical Manager - Excellence is responsible for driving quality outcomes, training and development, and performance excellence across the hospice team. This role oversees the Team Leaders and After-Hours team and serves as the primary clinical lead for all initiatives related to our hospice star ratings, quality improvement, clinical education, and onboarding. With a sharp focus on mentoring, compliance, consistency, and innovation, this leader ensures that Lifespark Hospice not only meets but exceeds industry standards. The Clinical Manager - Excellence will be accountable for developing and facilitating training programs, leading in-services, and maintaining an environment of continuous learning and professional growth.

Position Functions & Responsibilities:



  • Provides clinical leadership and oversight to Team Leaders and After-Hours team members.
  • Drives performance improvement initiatives aimed at increasing quality outcomes and star ratings.
  • Leads education and training programs, including onboarding for new hires and ongoing professional development.
  • Audits clinical documentation for accuracy, compliance, and quality standards; provides feedback and coaching based on audit results.
  • Develops and facilitates in-services and staff education sessions across all disciplines.
  • Serves as a mentor and resource for clinical staff, supporting evidence-based practice and hospice best practices.
  • Designs and implements structured orientation programs and individualized learning plans.
  • Collaborates with the interdisciplinary team, intake, and leadership to promote clinical excellence and service delivery.
  • Monitors and interprets quality metrics, satisfaction surveys, and other performance indicators to guide quality improvement.
  • Participates in QAPI meetings and leads action plans aligned with regulatory standards and agency benchmarks.
  • Collaborates with leadership to identify training gaps, address clinical deficiencies, and develop professional growth pathways.
  • Participates in and supports compliance audits, survey preparedness, and mock surveys.
  • Supports and reinforces a culture of accountability, professionalism, and clinical integrity.
  • Provides coverage and leadership support during MOD rotations or other operational needs.



Qualifications:



  • Registered Nurse (RN) licensed in the state of Minnesota.
  • Bachelor's Degree in Nursing preferred.
  • Certified Hospice and Palliative Nurse (CHPN) strongly preferred.
  • Minimum of five (5) years of hospice nursing experience required.
  • Minimum of two (2) years of experience in education, training, or clinical leadership roles.
  • Demonstrated expertise in quality improvement, staff development, and regulatory compliance.
