Position: Clinic Coordinator - LPN for Population Health

Wage Range: $50,000-$60,000 annually, depending on experience

Service Area: Lifespark Corporate Office, St. Louis Park 55416 (this is an in-person position, providing telephone and virtual care to our clients)

Schedule: 1.0 FTE, Monday - Friday 8:00am-5:00pm

The Clinic Coordinator-LPN is primarily responsible for day-to-day clinic operations and is the primary central communication and clinical operations contact for the field based primary care health services staff (ie: physicians, Life Managers, nurse practitioners or physician assistants) in the member's place of residence. The Clinic Coordinator-LPN will support clinical staff in the delivery of whole person senior primary care services including primary care management, supporting the LM/NP/MD team, and collaborating to deliver the Lifespark Experience to a geriatric patient population. The Clinic Coordinator-LPN will be traveling to our community client's home for phlebotomy and client needs.

Position Functions & Responsibilities:



  • Performs administrative activities including data entry, scheduling, patient registration, insurance verification, and reporting
  • Actively participates in creating and implementing improvements to achieve clinical, satisfaction and/or efficiency outcomes
  • Confirms and documents patient demographics and billing information.
  • Coordinates all requests for information including, but not limited to, death certificates, O2, orthotics, pharmacy requests, prior authorizations, etc
  • Serves as a liaison between clinicians, members, vendors, and other health care professionals
  • Responds to inquiries by telephone, email, or fax
  • Responsible for validating and updating member's EMR chart, including transcription and entering orders in EMR for provider to sign
  • Maintains a strict standard of confidentiality to ensure our client's protected health information (PHI) is secure
  • Attends regularly scheduled training sessions and professional development courses as required
  • Submit necessary pre or post authorization paperwork to payers
  • Provide Phlebotomy to our community clients in their home
  • Perform vital signs and assessments for the community clients in their home
  • Care Coordination for the community clients

  • Effectively articulate and differentiate the Lifespark Life Experience Alternative Delivery System
  • Provide assurance and relief (peace of mind) to clients and families that Lifespark will effectively address their needs and wants.
  • Make the clients feel comfortable and well-cared for.
  • Establish Lifespark as a thoughtful, professional company that has a well-designed approach to helping clients and families (i.e., we have our act together).
  • Serve as a client advocate and liaison working with outside resources (therapy, DME, hospice, guardian, healthcare providers, caregivers, trusted advisors) for fulfill needs and work with Lifespark Navigation to provide the right services and the right time (7 Element Partners and Resources).
  • Completes all documentation in a timely manner and responsible for client record content
  • Complete orientation and annual training requirements.
  • Promote safety in the workplace through real time coaching, participation in safety awareness training and progressive discipline where necessary
  • Participate in weekend coverage as needed
  • Other duties as assigned



Qualifications:



  • Licensed Practical Nurse diploma required
  • Experience using technology including: Microsoft Office, Smart Phone, Calendaring, Clinical Software platforms (EMR), Wi-Fi, Printers, and Internet Browsing
  • Previous clinic experience highly recommended; Medical Assistant preferred
  • Must possess:

    • Ability to be organized and prioritize multiple demands in a fast-paced environment

    • Willing to travel to community clients for phlebotomy and assessments in the 7-county metro
