Are you a passionate RN with a heart for women's and children's health? Do you love connecting with people, building trust, and guiding families through life-changing experiences? If you're ready to blend clinical expertise with community connection, we have the perfect opportunity for you. As a Women's Health Program Navigator RN, you'll be the heartbeat of our women's health program-connecting expectant moms and their newborns to the care and support they need from the third trimester through postpartum and beyond. Reporting to our Senior Director (or their designee), you'll design, lead, and continuously enhance our Mom-Baby outreach and follow-up program. You'll be a friendly face in our clinics and on our hospital floors, helping moms navigate OB and pediatric care, eliminating barriers to care, and ensuring no one falls through the cracks. You'll coordinate appointments, follow up with families after delivery, and partner with clinical teams to provide a warm handoff between services. Think of it as part care coordinator, part advocate, part educator-and all heart. What You'll Do:
