Overview

The Hospice Chaplain provides spiritual care and services based upon initial and ongoing assessments of the patient and family. Actively participates within the Interdisciplinary Team (IDT) providing consultation and education to team members and families about spirituality and related care and services. Implements and coordinates the provision of spiritual care to alleviate distress and existential loneliness. Hours may vary depending upon census and program need.

Responsibilities

Assists patient/family in mobilizing spiritual resources to enhance coping and quality in end of life experience.
Clarifies value issues with patients, families and staff to support ethical practices/decisions.
Demonstrates awareness of potential and unintended influence over patients who are vulnerable due to their illness, and protects patients and family against conscious or unconscious proselytizing.
Designs and implements individualized spiritual services and ceremonies of faith/worship and ritual.
Details (direct or incidental) possible to enhance service/care is communicated across service lines and among disciplines.
Develops a spiritual care plan to prevent and/or minimize spiritual distress and preserve/family beliefs. Evaluates effectiveness of spirutal services and individualizes spiritual plans of care and modifies interventions as indicated.
Documents spiritual assessments, interventions and spiritual services within patients electronic medical record.
Facilitates interdisciplinary consults to optimize and individualize patient treatment.
Identifies those at risk for religious/spiritual conflicts that compromise or detract from end of life quality and end life issue resolution.
Interprets and communicates pt/family faith and culture traditions. Identifies and communicates when spiritual/religious beliefs may impact the physical and psychosocial care provided by other team members.
Investigates and applies payer specific Commercial Hospice Benefits to optimize treatment opportunities.
Makes presentations concerning spirituality, religion and end of life care as requested by hospice leadership.
Provides care according to plan of care/orders. Recertifications are prepared with chaplain orders for patients actively served.
Provides education and resources to patients, families and significant others.

Compensation and Benefits

The compensation range for this position is $23.97per hour - $40.03per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
