Do you have a passion for our community and the skill set to support growth at an up-and-coming health organization? Progressive Community Health Centers is seeking an experienced professional to serve as a Social Worker at our Lisbon Avenue Health Center. Under the direct supervision of the Director, Behavioral Health and supported by the Behavioral Health Consultant (BHC) team, the Social Worker is responsible for managing, educating, and referring high-risk patients with chronic illness or disease.

Founded in 1999, Progressive Community Health Centers is an independent non-profit organization that operates four clinic locations in Milwaukee, Wisconsin. More than 100 employees support our agency's mission to improve the health and quality of life of the community by providing culturally competent services that address identified needs. We provide high-quality, comprehensive care to roughly 16,000 patients annually majority of who are low-income. No one is ever turned away from our clinics based on insurance status or ability to pay. We value a patient-centered philosophy and provide a range of services that include family medicine, internal medicine, women's health, urgent care, dental, radiology and integrated behavioral health. We also offer supportive services such as case management, health education, and benefits enrollment.
What will you do?
The social worker is responsible for assisting Progressive Community Health Centers (PCHC) patients by creating linkages to care and providing education and self-management support. Care linkages can include navigation to social support agencies based on identified needs or resources to mitigate the impact of social determinants of health. Schedule: Monday - Friday, 8 a.m. - 5 p.m. Responsibilities

It is mandatory that all employees, employed providers and volunteers are vaccinated. All hired candidates must submit proof of vaccination prior to their orientation.
