Job Objective: RWJBarnabas Health is seeking a highly dedicated & experienced RN Clinical Navigator at Monmouth Medical Center. The Clinical Navigator is a Registered Nurse with experience in oncology services who navigates patients throughout the process of the continuum of cancer care from diagnosis, treatment, to survivorship in collaboration with the multidisciplinary team, and according to agreed clinical guidelines, with the objective of coordinating high quality patient care across all care delivery sites, Functions on the multidisciplinary team as an advocate, interpreter, educator, and counselor for the oncology patients, Responsible for ensuring all adult patients with an oncology diagnosis receive quality and comprehensive services, Serve as a clinical resource in hematology/oncology care management, Serves as a single point of contact/liaison throughout the facility- for referring physicians, patients, caregivers, and in the community regarding services provided for the identified patient population, Provide expert nursing care, which includes: direct clinical practice, consultation, education, care coordination, education and research, The Clinical Navigator is a Registered Nurse who can provide nursing care in the health care system within the scope of practice defined by the Nurse Practice Act and regulatory standards and assures awareness of ethical standards of practice as defined by the ANA code of ethics, The Clinical Navigator ensures safety in practice and is inclusive of the principles of evidence based practice by maintaining a realization of own individual level of knowledge and experience and assumes responsibility for advancing knowledge of self and others of the patient care team by communication with and utilizing resource persons and programs available, Is responsible for reporting identified safety issues such as hazardous environments ie, damaged floors/walls/ceiling tiles/unsecured areas and medical errors, Is responsible for knowledge of the patient population served, Responsible for supervising and directing the delegation of tasks to appropriate team members and collaborates with other patient care providers, Responsible for consistently setting and evaluating priorities through appropriate interventions with measurable outcomes toward improvement, The Patient Navigator utilizes the nursing process to integrate all aspects of the provision of care and individualizes the plan of care to address a specific patient's needs, BSN preferred, Completion of Clinical experience in NLN accredited school preferred, Current NJ RN Licensure required, Oncology experience required, BLS required, Oncology Certified Nurse - National Certification preferred, Patient Navigator Certification preferred Requires leadership, organizational, written, and verbal communication and excellent interpersonal skills Job Specific Requirements Assists patients in understanding their diagnosis, treatment, options, and the resources available including educating eligible patients about appropriate clinical research studies and technologies, Communicates with all members of the healthcare team, as appropriate about patient/family needs and concerns within appropriate time frames, Coordinate multidisciplinary planning conferences, develop concise patient summaries for use by the care team, and document recommendations made utilizing standardized care protocols in accordance with nationally recognized guidelines, Deliver quarterly report to Program Director and other groups which demonstrates outcomes and performance improvement activities, Demonstrates the knowledge, skill, and coordination to provide the nursing care and guidance to the cancer patients from screening to survivors Facilitate appointments for consults and support services within established service standards, including social workers, dieticians, physicians, etc Follow patients throughout the care continuum, including same-day surgery unit, and inpatient admission, and collaborate with inpatient care management resources Functions in an organized and time conscious manner Initiates and documents patient teaching including family and significant others based on assessment of needs, Initiates and performs ongoing review of policies related to service provided, Where appropriate, updates or writes new policies to enhance professional practice, Maintains the standards of nursing care and implements the policies and procedures of the hospital and Nursing Departments Partners with patients , families, the interdisciplinary team, and community resources to provide well- coordinated, timely, compassionate, exemplary, interdisciplinary care, Serves as an essential link between patients and all other care providers Provides education and information to the patient and family, helping to make the care seamless, continuous and comprehensive, Responds to patient request for information regarding the disease process, expected side effects of treatment and community resources, Serves as a resource for community educational events, such as health fairs, screenings, symposiums, and lectures as well as staff education Supports the patients during difficult decision-making periods, Assists in coordination of end-of-life care for patient and family and provides emotional support Systematically and continually performs the functions of assessing, planning, implementing, and evaluating the care according to the nursing process and Oncology Nursing Society Standards of Practice, Work with Cancer Registry staff and others to collect data, track outcomes, and support strategic planning process Other Duties: Please note this job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employee for this job.
