POSITION SUMMARY: Under the supervision of the Pharmacy Technician Supervisor, and according to department policies and procedures and guidelines, the Medication Reconciliation Technician will assist pharmacists in obtaining a list of the most accurate prior to admission medications by interviewing patients and consulting with their outpatient pharmacies. The Medication Reconciliation Technician will obtain medication histories in the Emergency Department as well as in the inpatient areas. Through performing medication histories, the Medication Reconciliation Technician plays an integral role in helping to prevent medication errors caused by inaccurate or incomplete information. The technician will be expected to provide exemplary service to other internal customers at BMC, as well as embody BMC's values in all interactions with patients and colleagues. This position will rotate through days, evenings, weekends, and holidays.
