We are pleased to present the following position for your consideration: Overview: Heritage Behavioral Health Center is seeking a full-time LPN position in our Integrated Health Clinic. This position will provide primary and psychiatric health services to individuals with a diagnosis of a serious mental illness and chronic health condition(s). This is a full-time position requiring a commitment of 40 hours per week, Monday through Friday, from 8:00 AM to 5:00 PM. Core Responsibilities Include:
- Provide direct nursing support to a psychiatric provider in our DeWitt County clinic two days per week, including patient assessments, documentation, medication administration, and coordination of care.
- Deliver nursing services in community settings three days per week, including our four group homes and client residences, with a focus on well-being checks, basic nursing assessments, medication education, and long-acting injectable administration.
- Accurately obtain and document vital signs, physical observations, and health-related concerns across all assigned settings.
- Administer prescribed medications (weekly or as ordered), monitor for adverse reactions, and report concerns promptly to the appropriate medical staff.
- Conduct medication monitoring and education to support safe and effective medication use by clients in the community.
- Monitor client health status and ensure timely, accurate, and complete documentation in the electronic health record.
- Communicate clearly and effectively with providers regarding changes in patient condition, urgent needs, or follow-up care requirements.
- Build positive relationships with clients, families, and residential staff, promoting a respectful and supportive care environment.
- Ensure supplies, medications, and nursing tools are organized and available for both clinic and community use.
- Coordinate with the pharmacy for timely refills, medication adjustments, and medication availability across multiple sites.
- Facilitate referrals and linkage to appropriate medical or behavioral health services as needed.
- Collaborate with team members to address urgent medical needs, provide triage support, and ensure clients receive appropriate care in a timely manner.
Knowledge, Skills and Abilities:
- Ability to engage individuals and decrease barriers to accessing treatment
- Ability to solve problems and multi-task and coordinate a high level of activities.
- Considerable interpersonal as well as oral and written communication skills
- Ability to work in a team-based environment
- Strong customer service skills
- Ability to respond effectively in an emergency situation for patient care, possess strong listening and communication skills, and be attentive to detail.
Education and Experience:
- Active undisciplined State of Illinois Licensed Practical Nurse (LPN) licensure.
- Excellent organizational skills and attention to detail.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate and empathetic demeanor with a commitment to patient-centered care.
Salary Range: $28 - $37 an hour, dependent on education and experience (Also $2,500 sign-on bonus option) Heritage also offers the following with this position:
