Community Health Nurse - Chronic Disease Management & Prevention Empower communities. Advocate for wellness. Lead transformative health education. Position Summary: As a Community Health Nurse specializing in chronic disease management and prevention, you will serve as a subject matter expert within the Community Health Strategies team. Your focus will be to develop, implement, and support impactful health education and outreach programs aimed at reducing risk factors and improving quality of life for individuals living with chronic conditions. Through culturally competent engagement, you'll help bridge gaps in healthcare access, provide motivational health coaching, and collaborate across departments and community organizations to build healthier, stronger populations. Key Responsibilities:
Lead and coordinate community outreach and education programs focused on chronic disease risk reduction and symptom management Serve as a community-based advocate and coach to help individuals navigate the healthcare system and reach personal health goals Deliver culturally relevant health education, enrollment support, and wellness programming Actively engage in initiatives related to nutrition, nourishment, and community health needs assessments Build and maintain strategic partnerships with internal teams and external organizations Represent the organization in community events and educational forums through public speaking and health promotion
Minimum Requirements:
Bachelor of Science in Nursing (BSN) Licensed RN in the state of Missouri 5-7 years' experience in program and/or project management Mastery in public speaking, motivational interviewing, and solution-focused coaching Strong written and interpersonal communication skills Experience in community or public health nursing Personally committed to wellness-actively pursuing your own health goals Proficient in Microsoft Office Suite Able to work a flexible schedule, including some evenings and weekends If required to drive, must meet Fleet Driving Safety compliance and hold a current, valid Class E license in Missouri or Kansas
Preferred Qualifications:
Bilingual in Spanish and English Certified Diabetes Educator (CDE) Certified Wellness Coach, ACSM Health Fitness Specialist, or CHES (Certified Health Education Specialist)
