*$55K/yr. Responsible for (1) ensuring a seamless transition for consumers discharging from CrisisCare Center into outpatient services. (2) Responsible for the case management needs of consumers while at the CrisisCare Center. (3) Work in partnership with team members across the FCS behavioral health care system. Requirements
