Care Coordinator Nassau County

RESPONSIBILITIES:

• Schedule appropriate staff based on competency and availability to provide quality care to clients
• Function as facilitator among clients, families, clinical staff, case managers and insurance companies
• Informs patient and/or significant in delay or reschedule of service times when required
• Receives and communicates customer/patient complaints to supervisors, as necessary
• Communicates recruiting needs to administrator/recruiter and participates in recruitment, hiring and orientation activities
• Coaches and counsels para-professional staff
• Ensures para-professional compliance with agency policies
• Creates and maintains all new patient files
• Maintains and updates scheduling lists
• Answers telephone and responds to inquiries by directing calls to appropriate staff
• Participates in agency On-Call as needed
• Participates in annual administrative areas of performance evaluation for the HHA/PCA
• Participates in verification of time slips for payroll and program purposes weekly
• Other administrative assignments as directed to facilitate daily operations

QUALIFICATIONS:

* High School Diploma or GED

* Some College or business education preferred.

* Minimum of two years of office experience work in a health care setting.

* Bilingual preferred

* Ability to communicate effectively with clients, families, caregivers and administrative staff.

* Demonstrates self-motivation, self-direction, organizational skills, flexibility and the ability to deal with a high level of stress.

* Good telephone, communication and interpersonal skills.

* Organizational skills and ability to maintain accurate schedules, notes and records.

* Proficiency in the use of computers and software programs

* Ability to apply common sense understanding to carry out simple one or two-step instructions.

* Ideal candidate will have experience as a Care Coordinator for a NY State Licensed Agency
