SCOPE:


Under general supervision, the Pharmacist I compounds and dispenses medications, drugs, and other pharmaceuticals according to professional standards and practices. This entry-level position involves the preparation and maintenance of all necessary records on prescriptions, patient charges, and inventory. The Pharmacist consults with nursing and medical staff regarding prescriptions, orders, drug and pharmaceutical details, patient reactions, errors, and complaints. The role includes initiating and following quality assurance and safety policies and procedures and monitoring compliance with all legal regulations and accreditation organizations. The Pharmacist supports and adheres to the US Oncology Compliance Program, including the Code of Ethics and Business Standards.


Responsibilities

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Prepares and dispenses medication orders.
  • Maintains and evaluates patient profiles and comprehensive drug data files to assess orders for any incompatibilities.
  • Checks all orders for completeness of information, verifies patient information, and proper physician authorization.
  • Ensures insurance approval before admixing. Admixes drugs as ordered by physicians accurately and timely per department guidelines and prepares all drugs within OSHA and interoffice guidelines.
  • Ensures correct preparation of all formulas or medications.
  • Checks all prescriptions for completeness, calculates correct dosages, converting between metric and apothecary equivalents.
  • Organizes and directs technician workflow; verifies their preparation and labeling of pharmaceuticals; verifies order entries, charges, and inspections.
  • Provides pharmacological information by answering questions and requests from healthcare professionals and patients.
  • Provides and documents drug counseling to all patients.
  • Ensures adherence to all legal and professional requirements, including those for controlled substances.
  • Consults with patients to determine pre-existing conditions to assess medication compatibility.
  • Instructs patients on the effects of food and alcohol intake, warns of potential side effects, and provides professional drug information and assistance to medical staff.
  • Participates in pharmacy teaching and in-service programs.
  • Properly disposes of all materials used when mixing chemo or non-chemo IVs.
  • Logs and dates all drugs in use, checks drugs monthly for outdates, and keeps accurate temperature logs.
  • Maintains accurate logs on all protocol drugs and patient assistance drugs.
  • Assists with inventory control of medications, orders supplies and medications, and receives and shelves medications.
  • Ensures standard drug supplies are adequately stocked in all departments.
  • Prepares labels, files, and prepares billing, and computes prices.
  • Receives new drug education from drug representatives, staying abreast of new information.
  • Maintains confidentiality of all information related to patients and medical staff.
  • Prepares reports and monitoring activities as directed.

Qualifications

MINIMUM QUALIFICATIONS:

  • Bachelor's degree from an accredited school of pharmacy.
  • Entry-level position.
  • Oncology background preferred.
  • Current license as a Pharmacist in the state of practice.
  • May supervise the activities of pharmacy technicians.
