Central City Concern is an innovative nonprofit agency providing comprehensive services to single adults and families in the Portland metro area who are impacted by homelessness, poverty and addictions. The Behavioral Health Assistant (BHA) works as a member of a multi-disciplinary behavioral health care team providing integrated behavioral health services to clients seen at Old Town Recovery Center. The BHA is responsible for providing administrative support to the behavioral health home, ensuring care that is accountable, comprehensive, proactive, well-coordinated and patient-centered. The Care Team addresses all treatment needs, including crisis management, substance abuse treatment, psychiatric evaluation & treatment, physical health treatment, mental health treatment, accessing benefits and entitlements, accessing appropriate and affordable housing. The Care Team is responsible for proactively managing the healthcare needs of the patients on their assigned team, including medical, mental health, addiction, and health behavior needs. This position has access to mental health billing and service records. Schedule: Monday - Friday, 8am-4:30pm Location: Old Town Recovery Center (33 NW Broadway, Portland, OR 97209) Compensation: Starting at $23.36/hr for QMHA-R, $25.76/hr for QMHA Seniority Bid Window Closes: 07/09/2025 Close Date: until filled MINIMUM QUALIFICATIONS:
- Bachelor's degree in related field OR 3 years relevant work experience.
- Must be registered, at minimum, with MHACBO to meet QMHA-R requirements.
- Experience working with homeless, low-income, or other vulnerable populations in a supportive role preferred.
- Has the competencies necessary to:
- Communicate effectively orally and in writing;
- Understand healthcare related terminology;
- Provide outreach, engagement and supportive services

- Must meet CCC privileging requirements as required by FTCA.
- Must have current CPR certification prior to start date.
- Must pass a pre-employment drug screen, TB test, and background check. This includes clearance by the DHS Background Check Unit.
- Physical ability to bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects. Must be able to climb stairs several times a day.
- Sufficient manual dexterity and physical ability to perform assigned tasks.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Prepare for daily staffing of clients, coordinate care for clients seen same day/week.
- Support care team & clients by participating in triage & evaluation of client needs.
- Obtain, process, and document all necessary information and paperwork required to assist the Care Team Coordinator (CTC) in the provision of health services to Old Town Recovery Center patients.
- Assists the CTC in tracking & organizing data in order to identify needs of the population; participates in follow up.
- Provide follow up contact in person or by phone to patients identified by CTM or other care team members for support and management of health related issues.
- Participate in outreach activities necessary to offer services and engage participation in a non-coercive manner. This includes outreach to patients who are due for routine preventive services, patients needing follow up for chronic disease management, and patients who have no-showed for appointments.
- Collect information regarding clients' health history, current health behaviors, and bio/psycho/social needs through a combination of direct interviewing, records review, and consultation with outside sources (after obtaining written consent).
- Participate in clinical staff meetings and case conferences with team members in order to utilize interdisciplinary expertise and facilitate individual treatment plans for clients.
- Provide educational information and materials to patients regarding available services in the community and how to appropriately access them.
- Coordinate services with all members of the care team and the client's support network.
- Maintain client records as required by program standards and funding requirements.
- Participate in training opportunities to advance skills and knowledge in addictions and mental health.
- Attend required meetings and mandatory CCC trainings in a timely manner.
- Works within scope of training and experience and seeks consultation when appropriate.
- Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
DESCRIPTION OF OTHER DUTIES:
- Connect clients with needed and available community resources. Follow-up with clients and agencies as appropriate to document use/success of referral.
- Assist clients in completing all appropriate forms for obtaining housing.
- Perform other duties as assigned.

- Knowledge of de-escalation methods or ability to be trained in de-escalation methods.

- Ability to work courteously and effectively communicate with the general public, clients, medical personnel, corrections personnel, police, and co-workers.
- Ability to maintain accurate records (paper and electronic) and appropriately complete and route necessary paperwork.
