Resources for Human Development seek mission-driven Case Managers to join our program in the Philadelphia area. The case manager will be directly responsible with a case load working with men and addressing needs related to each participant as needed. The case manager will work with direct staff for the ease of services and communication here at RHD Fernwood.

This position offers a limited time offer of $2,000 Sign-On Bonus (Split between the first year of employment with RHD Advantages of Working with RHD as a full time employee.

Essential Duties and Functions

The position requires the following responsibilities and duties as a case manager:

- Creates effective service plans applicable to the participant themselves

- Data entry of all case notes for participants

- Demonstrate the ability to follow all RHD and OHS rules and regulations

- Assist in providing resources and completing paperwork with participants as required

- Assist participants with money management

- Other duties as assigned

Qualifications

As a case manager, the job requires experience in the following:

- Ability to use Microsoft Office

- Ability to problem solve cases individually and/or within a team setting

- Knowledge of case management services working with a homeless population

- Excellent written and verbal communication skills

- Required to pass federal and state criminal background checks

Education/Experience

Bachelor's Degree in Social Work or Human Services related field such as but not limited to Sociology, Criminal Justice, and Psychology.

At least one year experience in Case Management or relative experience roles in the human services field (preferable).

Program Summary

Fernwood is a program with RHD's Philadelphia Behavioral Health and Housing that provides emergency housing services for homeless men year-round according to our funder, Philadelphia's Office of Homeless Services (OHS). The Case Manager will work with one of the following emergency housing Units managed by Resources for Human Development (RHD). During the stay of the homeless men focus on the five domains of life: housing, employment, health, community involvement, and education. The program's main goal is for participants to obtain permanent housing quickly.
