Job Description Summary: The People Concern's Malibu Outreach Team is seeking a Case Manager to join our multidisciplinary outreach team. This team is part of a City of Malibu strategy to combat homelessness. The goal of the Malibu Outreach Team is to identify, engage, and connect or re-connect homeless individuals to interim and/or permanent housing and supportive services. The team works closely with each client to develop individualized service plans. The team helps enroll clients in public benefits (i.e. General Relief, SSI/SSDI, Medi-Cal), submit housing applications, and access appropriate medical and behavioral health treatment. The Malibu Outreach Case Manager performs a variety of tasks to assist the most vulnerable and chronically homeless individuals in Malibu. The Outreach Case Manager is responsible for identifying, engaging and providing support to homeless clients, including providing referrals, linkages, and coordinating care. The Outreach Case Manager will model competence in wellness, recovery, and coping, and is responsible for maintaining the highest quality services and efficiency of operations. Job Description Essential Duties and Responsibilities:
- Work as part of an interdisciplinary team to engage and provide field-based services, meeting currently homeless clients where they are in order to eliminate barriers to services.
- Provide intensive case management services, linking individuals who are homeless to the range of integrated, comprehensive services provided by The People Concern, such as linkage to benefits, mental health and substance abuse services, temporary and permanent supportive housing, and other case management services.
- Serve as liaison with members of the Malibu community, including residents, business owners, and member of the faith community providing information and education about homeless issues and available resources.
- Engage clients in developing mutually agreed-upon goals documented in each client's confidential service plan.
- Provide initial contact, needs assessment, information, education, and referrals to unhoused clients.
- Attend Malibu community stakeholder meetings (along with other team members) and discuss progress and challenges.
- Work in collaboration with other team members to remove barriers to permanent housing; e.g., acquisition of identification, legal aid, benefits assistance, life skills acquisition.
- Provide case management and aftercare/follow-up services to clients placed in permanent housing.
- Work collaboratively with public, private, and nonprofit partners to provide services, including medical and psychiatric providers, law enforcement, fire, and paramedics.
- Adhere to an emphasis on customer choice, harm reduction, and psycho-social rehabilitation.
- Assure that all member paperwork and data collection is complete and up to date in the case management database.

Job Description Qualifications:
- High School Diploma or GED or Equivalent
- Demonstrated knowledge of and experience with the issues faced by the population served (homelessness, mental illness, substance abuse, medical issues, and domestic violence).
- Skilled in non-violent crisis intervention.
- Able to work in a high tolerance Harm Reduction model with clients experiencing multiple barriers.
- Computer literate; able to use computerized database system for information management.
- Able to maintain an effective working relationship with Malibu community partners (including police, firefighters, city staff, etc.) and maintain and build professional rapport.
- Valid CA Driver's license, reliable car, auto insurance, and an acceptable driving record

- Rugged field and indoor office environment.
- Regularly walk or drive to different sites across Malibu.
- Regularly required to sit, stand, bend and occasionally lift or carry up to 35 pounds.

- Will be exposed to elements like cold, heat, dust, noise, and odor.
- May need to bend, stoop, twist, and sit throughout the day.
