Job Roles

  • Under direction of Quality Improvement leadership, work with clinic staff and leaders to support implementation of quality improvement projects. Provide in person training and support to implement and sustain new workflows.
  • Assist with developing new educational resources and materials for patients and clinic staff to assist with closing care gaps.
  • Complete patient chart reviews in EHR and CMIS to monitor for compliance and identify trends in documentation.
  • Participate in developing pilot projects to support quality improvement and innovation initiatives across departments.
  • Provide education on quality measures and care gap closure to clinical and non-clinical staff in support of quality improvement initiatives.
  • Act within clinical scope to provide education or care to patients as part of quality improvement pilot programs (if applicable).
  • Develop and maintain professional relationships with clinic staff and leadership.
  • Maintain familiarity with quality measures from external agencies, such as NCQA HEDIS and HRSA UDS.
  • Performs other duties as assigned.

Education/Certifications/Licenses/Registrations

  • High school diploma or GED equivalent required.
  • Medical Assistant Diploma from an approved course required; bachelor's degree may be substituted in lieu of MA Diploma.
  • Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training.
  • Traveling between sites and other locations is required as an essential function of the job. Must have a car, a valid California driver's license, and proof of minimum levels of car insurance as required under California law, although limits of $100,000 are recommended. An acceptable driving record is also required. California law requires all drivers to obtain a valid California driver's license within ten days of establishing residency. Mileage and other reimbursement governed by policy.

Experience/Specialized skills (including Language)

  • At least 2 years' experience working in health care system with exposure to clinical quality measures.
  • Strong interpersonal and communication skills. Ability to provide education to staff and patients in manner that is professional and motivating.
  • Ability to work with multiple departments in-person and virtually.
  • Able to work well independently and within a team.
  • Ability to work on multiple projects and, with direction from supervisor, adapt to changing priorities.
  • Familiarity with quality measures and approaches to care gap closure.
  • Knowledge of electronic health record systems.
  • Familiarity in use of MS Excel, PowerPoint, and Word.
  • Bilingual in English and language that is predominantly used of underserved populations being serviced is preferred.

In the spirit of pay transparency, we are excited to share the base range for this position, exclusive of fringe benefits.

$28.00 - $34.00

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
