Job Summary: The Manager-Social Work Grant is a Licensed Clinical Social Worker in the state of California and provides clinical, administrative, human resource, and financial management for social work services inpatient and outpatient areas. Develops, implements and maintains a comprehensive and fiscally sound social services program and orchestrates cohesive patient care practices which address the psychosocial needs of the patients across the continuum of care. Ensures departmental operations are aligned with the goals, objectives, and key initiatives of the PATH Grant Enhanced Care Management Program (ECM). Maintains working knowledge of all applicable state and federal regulations, organizational policies, and national standards of practice. Mentors and coaches staff in transforming culture, processes, and practice that follows the patient centered care model and meets the demands of cutting-edge health care delivery. Performs other duties as needed. Education and Experience: Master's Degree from a School of Social Work accredited by the Council on Social Work Education required. Three years of Social Work experience that has provided an opportunity to demonstrate competencies in leadership, clinical social work, administration or social work education required. Knowledge and Skills: Broad knowledge of quality, patient safety, performance improvement, regulatory requirements, medical staff structure, peer review and legal aspects of care. Able to read; write legibly; speak in English with professional quality; use computer, printer, and software programs necessary to the position (e.g., Word, Excel, Outlook, PowerPoint). Able to relate and communicate positively, effectively, and professionally with others; provide leadership; be assertive and consistent in enforcing policies; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction. Able to communicate effectively in English in person, in writing, and on the telephone; think critically; work independently; perform basic math and statistical functions; manage multiple assignments; compose written material; work well under pressure; problem solve; organize and prioritize workload; recall information with accuracy; pay close attention to detail. Able to distinguish colors as necessary; hear sufficiently for general conversation in person and on the telephone and identify and distinguish various sounds associated with the workplace; see adequately to read computer screens, and written documents necessary to the position. Licensures and Certifications: Current Clinical Social Work License (LCSW) issued by the State of California, as evidence of ability to function as an independent, self-regulated professional practitioner of clinical social work required or in progress.
