Job Title: Care Coordination Specialist/Community Support Specialist - Part Time

Location: Trenton, NJ 08625

Pay Rate: $30.00/hr. On W2 without benefits

Duration: 6+ months

Work Hours: Flexible hours of approx 17.5-20hrs/week; LUNCH PERIOD: 1/2 hr

Job Description:

  • Responsible for routine telephonic follow up with every MFP participant who has recently transitioned from a nursing home, to identify barriers to independent living (e.g. Medicaid MLTSS service issues, housing, social isolation) and to seek resolution of those issues. Telephonic follow up with MFP participants who have returned to a nursing facility after transition to the community.
  • Complete follow up surveys in the MFP data management system and write detailed notes of the interaction. Follow up via email with Medicaid Managed Care Organization (MCO) to help resolve outstanding needs identified and ensure identified barriers have been resolved.
  • Identify trends or recurring service delivery issues reported by MFP participants. Provide monthly summary of outstanding needs in services identified by MFP participants.
  • Other related duties as assigned.


Justification:

  • This position is essential to ensure that people transitioning from nursing homes under the MFP/I Choose Home program are receiving quality services in the community and to identify gaps in services that could result in re-institutionalization.


Timeframe:

  • July 14, 2025- January 16, 2026 (26 weeks)


Required Qualification:

  • A bachelor's degree, and two (2) years of experience in the field of social services such as mental health, aging or disability services, health care setting, or human services agency. Familiarity utilizing electronic health records for documentation.
  • Spanish proficiency strongly preferred.
