Overview

Under the direction of the Hospice Clinical Manager, the Hospice Volunteer and Bereavement Coordinator is responsible for the day-to-day management, coordination, and oversight of the volunteer program, bereavement support, hospice intake and basic scheduling. Duties of the volunteer and bereavement coordinator include but are not limited to: recruitment, training, placement and supervision of all volunteers and bereavement mailings. In addition to those responsibility this position is responsible for hospice intake as well as administrative and secretarial support.
Qualifications

Certifications:

Proof of Auto Insurance - Varies;

Driver's License - Secretary of State (SOS),

Education: Associate's Degree,

Work Experience: 1year or more with Volunteer programs


Responsibilities

Develops a strong and thriving volunteer work force. Demonstrates the ability to successfully recruit and retain engaged and qualified volunteers. Provides volunteers with guidance, encouragement and coaching. Develops and implements a robust training program. Conducts both classroom and in home training and supervision. Maintains open lines of communication with volunteers, patients and family members to ensure that a high level of care and compassion is provided.Volunteer Recruitment: Demonstrates active and ongoing efforts to recruit volunteers. Plans and implements recruitment strategies that produce desired results (i.e. recruitment of specific skill, recruitment of specific geographically located volunteers and recruitment of specified number of volunteers). Evaluates and modifies recruitment strategies as indicated.Ensures all administrative, regulatory, and operational responsibilities are performed accurately and timely. Demonstrates organizational skills to successfully manage multiple priorities in a complex work environment. Assigns, coordinates, schedules, and supervises all volunteers in a collaboration with the interdisciplinary team (IDT) and the identified needs for volunteer and bereavement services. Responsible for all volunteer documents, including patient records, training hours, and other required documentation. Responsible for coordination in collaboration with social worker staff of all required and hospice specific bereavement function including but not limited to mailings. Ensure volunteers are trained appropriately to document per established policies. Participates as a member of the IDT. Maintains required records and competencies for all volunteers. Performs activities to assess ongoing evaluation of volunteer assignment and performance. Demonstrates excellent problem-solving skills.Develops and maintains a robust hospice program specific to support hospice program growth and other adjunct support therapies for the hospice patient and their support system. Routinely networks and environmentally scans the hospice industry for best practices of volunteer, bereavement and other supportive programs. Participates in community events (education, health fairs, marketing, publicity, grief programs).Ensures timely and accurate registration of patients by obtaining and processing demographic, financial, and clinical information. Utilizes various applications to obtain basic entry of demographic information. Follows guidelines for comprehensive and consistent entry of referral information. Monitors and acts upon information arriving to intake area via fax, telephone, or direct conversation with referral sourceProvides exceptional teamwork with intake, referral sources, office and clinical home care and hospice staff. Performs initial contact with patient/family for coordination of care through telephone calls to patients/families confirming their referral and scheduled admission visit. Participates in work group, office, and department team meetings. Coordinates and reassigns all hospice assessments.

Compensation and Benefits

The compensation range for this position is $19.25per hour - $31.19per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
