Job Title: Social Worker II - Dementia Navigator Department: Senior Services Summary: The Social Worker II supports members with dementia and their care partners by assessing care needs, facilitating transitions between care levels, and promoting social and emotional well-being. This role works closely with medical providers to ensure timely, appropriate, and compassionate care, while coordinating services and community resources to support members' safety and independence. Key Responsibilities:
Assess members' needs, including social, financial, emotional, and behavioral health.
Coordinate with medical teams to develop safe discharge and transition plans.
Conduct memory and behavioral health assessments using standard tools.
Provide crisis intervention and communicate with psychiatric providers as needed.
Refer members to community programs, protective services, or next-level care.
Document case management activities in designated systems (e.g., Salesforce, CSTARS).
Serve as liaison with payers and support seamless care transitions.
Collaborate with Club Memory Team and other stakeholders to improve quality of care.
Additional Duties:
Lead or participate in educational sessions and committee activities.
Maintain infection control and safety protocols.
Attend staff meetings and continuing education programs.
Qualifications:
Master's degree in Social Work, Sociology, Gerontology, or related field.
DC licensure as a Social Worker.
2-5 years of healthcare social work experience, with at least 1 year in case management or discharge planning.
Current CPR certification.
Skills:
Strong communication and crisis intervention skills.
Knowledge of dementia care, behavioral health, and community resources.
Understanding of age-specific care needs.
Ability to maintain confidentiality and work flexible hours.
Proficient in hospital information systems.
