Position Summary

The Pediatric Care Coordination Assistant (CCA) partners with the RN Care Manager and members of the interdisciplinary pediatric care team consisting of physicians, clinical professionals, and social workers to coordinate patient care, navigate patients and families through the health care system, and provide resources and support. This position will be a key component to achieving collaborative practices that exemplify MultiCare Health System's commitment to patient and family centered care, community engagement, and focus on transition of care for pediatric patients with physical and/or behavioral health needs.

Requirements


  • Post secondary coursework in a health related field or 1 year experience in clinical role such as medical assistant, certified nursing assistant or EMT
  • Bachelor's degree highly desirable
  • Experience with office organization and running reports or patient registries, helping with patient forms, and writing letters using excellent communication skills
  • Experience in a Pediatric Patient-Centered Medical Home highly desirable
  • Knowledge of local community resources, financial and quality issues affecting patient care, and Social Determinants of Health (SDOH) desirable
  • Community Health Worker experience desirable

Pay and Benefit Expectations

We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $20.07 - $28.87 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant's years of experience align.
