Primary Purpose Abstracts medical charts of Surgical ICU and Emergency General Surgery patients treated at PHHS to ensure accurate reporting of registry data to maintain accreditation and compliance with regulatory agencies. Education *Must have one year experience analyzing medical record charts; or equivalent combination of education and experience. Skills or Special Abilities *Must be able to demonstrate knowledge of anatomy and physiology concepts. *Must be able to demonstrate knowledge of medical terminology. *Must be able to interact well with physicians and other hospital staff on a daily basis. Must have excellent written and oral communication skills and be able to function with minimal supervision. *Must be able to demonstrate a working knowledge of PC and word processing, spreadsheet and database software. Must be able to learn ICD-9 codes. Responsibilities 1.Abstracts medical charts of Surgical ICU or Emergency General Surgery patients meeting registry inclusion criteria at PHHS. 2.Accurately records information including diagnosis, treatment and procedures into the appropriate repository to ensure proper record keeping of patient treatment. 3.Maintains patient tracking logs, forms and documentation that supports data repository. 4.Responsible for analyzing various admission reports to identify specific patient populations. 5.Maintains knowledge of applicable rules, regulations, policies, laws, and guidelines that impact data repositories. Develops effective internal controls that promote adherence to applicable state/federal laws, and the program requirements of accreditation agencies and federal, state, and private health plans. Seeks advice and guidance as necessary to ensure proper understanding. 6.Identifies ways to improve work processes and improve customer (internal as well as external) satisfaction. Makes recommendations to supervisor, implements and monitors results as appropriate in support of the overall goals and mission of the department, division and PHHS. 7.Stays abreast of the latest developments, advancements, and trends in surgical management by attending seminars and workshops, reading professional journals, and integrating this knowledge as appropriate into the job.
