JOB SUMMARY:
The Psychiatric LCSW-C is responsible to implement the plan of care for specific patients, beginning on admission and following through until discharge and insuring appropriate clinical intervention. Collaboration with the patient, family, Interdisciplinary Care Team members and external healthcare providers to assure safe, timely and efficient care is required. Initiates the plan of care and monitors its implementation. Functions as a member of the team in coordination of care to patients and serves as a problem solving resource. Position requires innovative skills for continuous improvement in patient outcomes and cost effectiveness. Internal communication is required outside of the work group to persuade or gain cooperation and acceptance of ideas.
MAJOR JOB RESPONSIBILITIES:
1. Employee demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on his/her unit as indicated by the following:
* Demonstrates ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs.
* Identifies the physical, motor, sensory, cognitive and psychosocial characteristics for age group(s) served.
* Demonstrates age appropriate communication skills.
* Demonstrates ability to provide the care needed as described in the job description and the department's scope of service for the patient groups under his/her care.
Age group(s) served will be one or more of the following: Infant (Birth to 12 months), Child (13 months to 12 years), Adolescent (13 years to 17 years), Adult (18 years to 69 years) or Geriatric (70 years and older).
2. Collaborates with Clinical Manager to develop therapeutic program for partial hospitalization.
3. Conducts group, individual and family therapy sessions for the partial hospitalization program and other psychiatric settings as needed.

4. Performs approved liaison, teaching, and outreach activities, and committee assignments which underscore professional practice and enhance the goals/objectives of the Hospital and department.
5. Represents the Hospital in collaboration with patients, family, the Care Team and Interdisciplinary Care Team to assure appropriate, safe, cost-effective, timely and efficient care.
6. Identifies patient and family needs and arranges for consultation with specialty services needed, including, but not limited to, medical, financial, environmental, procurement of services and discharge planning.
7. Serves as an advocate on behalf of patients and families. Communicates with patients to ensure understanding of third party payer guidelines.
8. Performs patient intake screenings and document per Hospital policy.
9. Acts as a liaison to external care providers and community health resources, participates in patient care conferences, assures completion of discharge-related functions on a timely basis and assures patient and family education needs are met.
10. Communicates and negotiates with appropriate third party payers to obtain necessary pre-certification.
11. Provides leadership and guidance to the care team in individualizing and implementing the plan of care.
12. Clinically evaluates psychiatric patients admitted through Emergency Department. Assist Emergency Department physicians and staff in maximizing efficiency in assessment and dispositional planning. Coordinate planning with 3rd party payors and treating/on call psychiatrist.
13. Provides crisis intervention as needed.
14. Provides ongoing assessment, clinical intervention and discharge planning coordination for patients admitted to psychiatric unit and partial hospitalization program.
15. Performs utilization management function in absence of Case Manager for psychiatry.
16. Provides consultation on the medical service for physician and staff regarding patients with psychiatric issues.

1. Performs duties in a manner that is consistent with the hospital's core values and the language of caring habits.
2. Attends mandatory departmental meetings, hospital in-services, and continuing education programs to keep informed of new developments affecting hospital initiatives and his/her area of expertise.
3. Participates on Continuous Improvement Teams as required. Performs tasks necessary for maintenance and improvement of his/her functional area within the department.
4. Maintains effective, timely lines of communication with his/her supervisor and co-workers.
5. Maintains acceptable attendance and punctuality in order to maximize his/her contribution to the organization in accordance with hospital policy.
6. Acknowledges his/her ultimate responsibility is to directly or indirectly serve the needs of every patient as defined in the HCGH Mission Statement.
Salary Range: Minimum $34.11/hour - Maximum $56.34/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
