About the Department Social Worker role that is deeply rooted in helping people and their families navigate the complex emotions and decisions around chronic illness and living well in their last phase of life. This role will support patients at Blodgett Hospital and continue to support them after discharge. Building trust and connection with patients and families to understand their fears, values, and hopes for the future. This will create a foundation for meaningful conversations about their care. Facilitating open dialogue to guide patients in discussions on their prognosis, chronicity, and potential outcomes with their medical care. Encouraging transparency in conversations between patients, families, and the care team so the patient is in control of their care and journey. Exploring what living well means to each person at this stage in their life. This could involve exploring their desires for quality of life, physical comfort, emotional wellbeing, spiritual, and autonomy in decision making. Support for families, loved ones and caregivers who may struggle with understanding the patient's chronicity of illness and accepting patient's wishes. Each patient will have a unique perspective on what living well means during their journey, and this role is crucial in helping them find clarity and peace in that process.
