Medical Center: Acute Care at Home (Full Time, Night Shift) - Our mission is to continue the teaching and healing ministry of Jesus Christ. The Clinical Nurse C-Acute Care at Home (ACAH), is a field nurse that provides age-specific and developmentally appropriate quality patient care in the home environment or in a similar alternate environment. Reviews ACAH criteria, patient's overall needs, level of care, admitting diagnosis, planned treatment with the emergency or admitting physician/nurse practitioner and readily accepts patient into ACAH program when approved. Utilizes discretion and independent judgment and performs the nursing process (assessment, care planning, intervention, evaluation) independently and in accordance with the established Professional Practice Model (Duffy's Quality Caring Theory) to achieve proficient individualized physical, psychosocial and spiritual wellness and provide compassionate bereavement support as appropriate. Meets qualifications, licensure, certifications and competencies as defined in the department-specific Plan for Providing Care. Applies theoretical concepts to clinical practice and exhibits self-motivation to continually expand knowledge base and clinical skills with a focus in acute care at home and specific patient population served. Inspires and adopts the transformation of evidence into point-of-care practice to optimize patient outcomes. Serves as a role model in patient care delivery methods and consistently demonstrates an advanced level of clinical practice, communication, and documentation. Functions within the expectations of an accomplished professional nurse and meets nursing excellence standards to maintain Magnet Recognition as outlined by the American Nurses Credentialing Center (ANCC). Represents nursing as an empowered profession and readily embraces new knowledge, innovations and improvements. Exhibits refined interpersonal skills and provides a positive influence to facilitate change and optimize patient satisfaction and outcomes. Engages in opportunities to directly and indirectly influence decision-making for clinical practice by supporting and participating in professional governance councils, committees, task forces and staff meetings. Contributes to a work environment of caring and cooperation among a culturally diverse workforce and patient population. Performs other duties as needed. Bachelor's Degree in Nursing required for external hires. Exception(s) may be granted as follows: 1) if currently enrolled in a nursing bridge program (AS degree to DNP degree), proof of 25% program completion required. DNP degree to be completed two and a half years post hire date. Exception to completion date may be granted for extenuating circumstances; 2) if employed in a LLUH entity with an Associate or Diploma Degree at time of application, AS or Diploma degree is acceptable. Minimum of two years' experience in nursing required. Experience in Home Health Care or transitional care preferred. The CN-C is eligible to be promoted into the CN-D classification when criteria outlined in the Clinical Advancement Program (CAP) handbook and job specifications outlined in CN-D job description are met. New hires are granted one year to meet CNC standards outlined in Clinical Advancement Program (CAP) handbook. Active California Registered Nurse Licensure required. Basic Life Support (BLS) certification issued by the American Heart Association required. Must be able to drive and have reliable transportation and a good driving record. Demonstrates advanced competencies in clinical nursing. Ability to apply theoretical concepts to clinical practice and continually expands knowledge base and clinical skills with a focus in area of specialty and population served. Basic knowledge of healthcare regulations and accreditation required. Exhibits professional behavior and appearance at all times and works calmly and responds courteously when under pressure. Substantial knowledge of state and federal regulations, finance, performance improvement, quality, patient safety, evidenced-based clinical practice standards, and legal aspects of care required. Knowledge of learning principles, and problem-solving methodologies required. Able to speak, read and write legibly in English (and Spanish preferred) with professional quality; use computer, printer, and software programs necessary to the position, e.g., Word, Excel, Outlook, PowerPoint, electronic medical record, electronic event reporting program; relate and communicate positively, effectively, and professionally with others; be assertive and consistent in following policies; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction; think critically; use sound judgment based on factual information and clinical knowledge; work independently with minimal supervision; meet deadlines; perform basic math and statistical functions; manage multiple assignments effectively; organize and prioritize workload; problem solve in a proactive and creative manner; recall information with accuracy; pay close attention to detail; hear sufficiently for general conversation in person and on the telephone; identify and distinguish various sounds associated with the work place; see adequately to read computer screens, medical records, and written documents necessary to position.
