Group Health Cooperative of South Central Wisconsin. BETTER TOGETHER 1.0 FTE/ 40 Hours per Week. Will consider hiring down to .8 FTE/32 Hours per Week. Position will split time between our Sauk Trails Clinic and East Clinic. The Diabetes Educator provides diabetes-related care and education to members with diabetes and coordinates care with practitioners, nursing and related health education staff. The incumbent incorporates clinical, educational and administrative components of a diabetes education program. The Diabetes Educator provides education and training on the pathophysiology of diabetes, modification of diet and exercise, monitoring/testing blood sugar levels, coping methods, problem solving, and risk reduction techniques. The Diabetes Educator also provides medication management following protocols under the guidance of Primary Care Physicians.
