1.0 FTE/ 40 Hours per Week. Will consider hiring down to .8 FTE/32 Hours per Week.

Position will split time between our Sauk Trails Clinic and East Clinic.

The Diabetes Educator provides diabetes-related care and education to members with diabetes and coordinates care with practitioners, nursing and related health education staff. The incumbent incorporates clinical, educational and administrative components of a diabetes education program. The Diabetes Educator provides education and training on the pathophysiology of diabetes, modification of diet and exercise, monitoring/testing blood sugar levels, coping methods, problem solving, and risk reduction techniques. The Diabetes Educator also provides medication management following protocols under the guidance of Primary Care Physicians.

JOB SPECIFIC MINIMUM QUALIFICATIONS



  1. Graduate of an accredited nursing or dietician program.
  2. State licensure as a Registered Nurse or Registered Dietician is required.
  3. Certification as a Diabetes Educator (CDCES) or working towards certification required. The incumbent must obtain certification within one year of employment.
  4. Two (2) years' experience managing the care of a diabetic population preferred.
  5. Possession of or eligibility for a valid Healthcare Provider CPR/AED certificate is required. The incumbent must obtain certification within four (4) months of employment.
  6. Ability to travel to various work sites. Incumbent is expected to provide personal transportation. Possession of a valid Wisconsin driver license if the employee is operating a motor vehicle.
  7. Knowledge of basic counseling skills and techniques.
  8. Ability to be culturally sensitive and work with a diverse population.
  9. Ability to perform patient assessment and design individualized patient action plans.
  10. Ability to coordinate patient care and follow-up.
  11. Ability to maintain patient confidentiality. Knowledge of HIPAA requirements.
  12. Ability to read, develop, write, explain and critique a variety of creative written patient education materials.
  13. Knowledge of and ability to use computers. Familiarity with Microsoft Office programs (Outlook, Teams, Word, Excel, SharePoint, etc). Ability to learn and use electronic medical record systems such as EPIC.
  14. Ability to work with clinic and administrative staff. Ability to be a team player in a team oriented environment. Ability to interact with staff at all levels.
  15. Excellent written and verbal communication and interpersonal skills. Ability to speak and listen in person and on the phone.
  16. Professional manners are necessary to deal courteously and effectively with patients and the public.
  17. Ability to work flexible hours including some early evening hours, as required.
