Description

The Team Lead position for Housing Supportive Services is responsible for coordinating and ensuring quality service delivery across all Housing services & programs. This role includes overseeing subsidy data collection, attending community meetings in the absence of the Program Manager, reporting, service coordination while maintaining a case load. The Team Lead will offer support to a diverse team working between multiple contracts under the Transitional Health Umbrella ensuring smooth collaboration with community resources. This position also involves orienting, training, and leading staff in Housing Supportive Services programs.


Responsibilities:

Program Coordination and Leadership



  • Coordinate services within Housing Supportive Services, ensuring timely and effective service delivery.
  • Maintain tracking systems for subsidies and other funding groups to monitor contract deliverables, create reports, and ensure compliance with quality standards.
  • Facilitate team meetings and collaborate with supervisors to manage program goals and support staff development, in absence of Program Manager.
  • Lead training for new staff and ongoing guidance for existing staff, ensuring compliance and high-quality service delivery.
  • Create and maintain a team resources for known community resources and updates.


Data Management and Compliance



  • Collect, track, and analyze data for reporting, ensuring accurate and timely documentation.
  • Perform data entry into systems such as Apricot Social Solutions, HMIS Clarity Software, REDCap, CareLogic, and others as required.
  • Conduct regular audits of staff files and databases to ensure adherence to program and contract guidelines.
  • Support Program Manager in ensuring all client records comply with contract requirements and quality assurance standards.


Client Services and Case Management



  • Provide direct case management and support to individuals in Housing Supportive Services.
  • Assist individuals with applying for benefits, learning daily living activities, psychosocial support, and service coordination.
  • Develop and maintain treatment plans or supportive plans to aide in clients' recovery and long-term goals.
  • Establish and maintain relationships with community resources to support client needs and offer support for successful housing retention.
  • Assist clients with navigating housing systems, including locating and maintaining permanent housing.


Community Outreach and Teamwork



  • Coordinate with other inter-agency programs and community organizations to build effective support systems for clients.
  • Conduct community presentations to promote programs and build partnerships.
  • Foster a collaborative work environment by helping resolve issues, supporting team members, and facilitating problem-solving.

Qualifications

Job Requirements:

Physical Requirements:

This position normally requires that physical demands of standing, walking, bending, lifting or performing other work requiring low physical exertion, talking and hearing on a regular basis to perform the job requirements. These physical demands are required up to 80% of the time. Position also requires ability to drive with adequate vision and skill.

Non-Physical Requirements:

Education



  • Lived Experience with Mental Health and/or Substance Use Disorders, Required
  • Bachelor's degree in Social Sciences, Education, or related field, preferred


Experience



  • 1-3 years of experience working with individuals experiencing an SUD or Mental Health challenge, preferred
  • 1 year in house support services


Special Skills



  • Strong case management, organizational, and communication skills (both verbal and written).
  • Ability to manage multiple tasks and deadlines while ensuring attention to detail and high-quality service delivery
  • Ability to work independently and collaboratively with minimal supervision.
  • Group facilitation and presentation skills
  • Bilingual (English/Spanish) preferred but not required.


Licensure, Registration, Certification



  • Valid Washington State driver's license and required minimum liability insurance for WA State
  • Must be deemed insurable as determined by Catholic Charities' liability insurance provider
  • First Aid/CPR Certification
  • Agency Affiliated Counselor Registration (if applicable)


Employment is conditional upon:



  • Being cleared by criminal background check and fingerprinting when required
