Responsibilities Job Summary The Pharmacy Medication Reconciliation Technician will improve medication safety by assisting pharmacists in collection and documentation of up-to-date medication lists for patients being admitted to the organizations hospitals in order to optimize the medication reconciliation process and reduce the number of medication errors secondary to inaccurate or incomplete information.
Qualifications Education High School Diploma or GED Required Experience <10 months pharmacy technician experience or completion of a formalized technician training program. Required Licenses and Certifications Certified Pharmacy Technician (CPT) Current license by AZ State Board of Pharmacy. Required Pharmacy Technician Certification Board (PTCB) Current Pharmacy Technician Certification (CPhT) issued by Pharmacy Technician Certification Board (PTCB). Required |
